You can use the Internal Review Application (Form A17) below to apply for internal review of a decision made under the Government Information (Public Access) Act 2009. You must lodge any application for review within 20 working days after you are notified of the decision. If you need help to fill out this form to apply for an internal review, please contact the Office of the Information Commissioner on 1800 INFOCOM (1800 463 626).
